The Erie County Health Department accepts applications for employment ONLY when there are positions available. When there are employment opportunities, the position is posted and applications are accepted for a minimum of three days unless noted in the posting. Please download the following application and all attached forms. These forms must be completed in order for your application to be considered. Completed Erie County Health Department application and forms may be faxed to (419) 626-8778, attention Human Resources Office. ALL Erie County Health Department applications and forms MUST be received by the Human Resources Office prior to the posted deadline. Resumes are accepted only as a supplement to the application. Applications will be considered for employment if received within the current posting timeframe.
